Physician Referral

Our goal at BCDI is to make it easy to partner with others in the medical community. At BCDI, we work with hospital systems and physician networks throughout the state of Illinois to provide comprehensive care for our patients. We know that working together for patients diagnosed with bleeding, blood clotting and other blood disorders creates the best possible outcomes for our patients.

Whenever a patient has a scheduled medical procedure, an emergency situation or a general doctor or dentist visit, the the BCDI team ensures that other physicians and caregivers know what to expect. This collaborative process allows us to care for our patients regardless of location or hospital system.

To refer a patient for one of our board-certified hematologists or physicians, you can follow any of the below options:

1. Call 309-692-5337 and speak to a scheduling assistant.
2. Download this referral form (link to provider referral form) and fax the completed form to 309-693-3913.
3. Complete the form below with the patient’s information.
